30 Day Unplanned Readmissions

This measure tracks patients returning to a hospital within 30 days of being discharged. Patients may have an unplanned readmission for any reason. After orthopedic surgery, a readmission could occur because of mechanical complications or to rule out a surgical site infection.

Our unique focus in orthopedics has allowed us to become a surgical center of excellence. Our operating rooms are designed specifically for orthopedics—from specialized teams using evidence-based protocols to sterilization technology and air handling designed to help stave off infection. Our patient-centered model of care ensures team collaboration, coordination, and safety measures for patients while they are in the Hospital.

A surgical episode extends beyond discharge. What happens to a patient once they leave the hospital is just as important to the outcome as the surgery itself. The patient and their family play a big role in making sure that they are following their rehabilitation plan. NEBH Case Managers work with each patient to ensure a smooth hand-off as patients leave the Hospital. They also stay in touch with the patient to monitor quality and ensure satisfaction with post-surgical care.
