Medical Records & Images

Medical Records

The Health Information Management Department maintains the medical records of all New England Baptist Hospital patients. We do not maintain records for private practice physicians or the Occupational Health Center.

For a copy of your Medical Record, please submit a letter of request including the following information:

  • Name (please print)
  • Date of birth
  • Information requested (dates of service, reports)
  • Your address
  • Your signature

NEBH has partnered with DICOM Grid to provide you with a simple and convenient way to share your images with your NEBH healthcare provider.

This secure, electronic sharing system offers significant benefits, including:

  • Reduced delays in your care—we send images directly to your doctor prior to an appointment.
  • Avoidance of increased cost due to repeat imaging resulting from lost or unreadable CDs/film.
  • Permanent storage in one convenient location—you’ll to never have to bring your CD to NEBH.
  • 24/7 access to your medical imaging exams and reports.

Authorization to Release Protected Health Information form

 

If you want your record sent to another party, you must complete our Authorization to Release Protected Health Information form.

This form meets HIPAA requirements (the Health Insurance Portability and Accountability Act of 1996 – 45 CFR Parts 160 and 164).

Please be sure to fill out the form completely and to allow sufficient time to process your request. If you need the record for an upcoming appointment, please specify your time frame to assist in expediting your request.

There is a charge for this service, unless your record is being sent directly to a physician. Please note, this fee is waived if sending the record abstract only.
