30 Day Unplanned Readmissions
This measure tracks patients returning to a hospital within 30 days of being discharged. Patients may have had an unplanned readmission for any reason. In orthopedic surgery, the most common reasons patients are readmitted to the hospital include pain and surgical site infection.

Our operating rooms are designed specifically for orthopedics from specialized teams using evidence-based protocols to sterilization technology and air handling designed to help stave off infection. Our patient centered model of care ensures team collaboration, coordination and safety measures for patients while they are in the hospital.
A surgical episode extends beyond discharge. What happens to a patient once they leave the hospital is just as important to the outcome as the surgery itself. The patient and their family play a big role in making sure that they are following their rehabilitation plan. NEBH Case Managers work with each patient to ensure a smooth hand-off as patients leave the hospital, often going home. They also stay in touch with the patient to ensure satisfaction with their post-surgical care and monitor quality.
